Soon after take-off, in 1989, a United Airlines flight lost its No. 2 engine, and the cockpit crew learned that not one but all three hydraulic systems had failed. This case study analyses how an event considered a ‘billion-to-one’ likelihood of occurring, DID occur, and raises questions as to how far do
we manage risk. It also portrays the role of across-the-board teamwork in emergency situations. The outcomes of this event also resulted in the alteration of all DC10s, which potentially also prevented the loss of many lives in the future.
